U.S. AIDS activists welcome London 'cure' but warn against complacency

Stem cell patient 'cured' of HIV | News London Patient Cured Of HIV/AIDS
Anna Jefferson | 07 March, 2019, 21:40

A "London patient" became the second man after the "Berlin patient", to be cured of HIV after stem cell therapy put their infections in remission without medication, a breakthrough that has given hope to 37 million people living with HIV, which causes AIDS. After chemotherapy, he had a stem cell transplant from a donor who was naturally resistant to HIV.

A second patient has reportedly been cured of HIV, which could be a major milestone, in the global fight against the AIDS epidemic. Gupta was cautious about using the word "cure" yet, noting that the man's HIV viral load could still rebound.

20 years ago, times were bleak for those diagnosed with the virus.

Brown, who is still HIV-free, told the Telegraph: "It gives the scientists hope that they should keep looking for finding cures for HIV and also gives patients hope that there will be a cure that will help them one day". "It's great to have an addition to my family - I consider him my sibling and I can't wait to meet him".

Both patients had received bone marrow transplants to treat blood cancers, receiving stem cells from donors with a rare genetic mutation that prevents HIV from taking hold. This indicates that other patients, in the same circumstances, should where possible receive transplants from a donor with this same gene mutation.

"The work we did involved cells that were susceptible to HIV, but even then we saw a dramatic reduction in the size of the HIV reservoir and a prolonged rebound time, suggesting that the transplant itself can reduce the burden of HIV", Henrich told aidsmap. If researchers can find a way to easily manipulate CCR5, perhaps with CRISPR gene editing, they could open up a path for H.I.V. patients to enter remission without undergoing invasive transplants. Extensive testing of his blood and T cells revealed undetectable HIV.

Stem cell transplants typically are harsh procedures which start with radiation or chemotherapy to damage the body's existing immune system and make room for a new one.

Nearly three years on, and more than 18 months after coming off antiretroviral drugs, highly sensitive tests show no signs of the man's previous HIV infection.

Many patients now have good control of their disease with treatments such as Gilead's anti-retroviral drug Biktarvy, which combines three HIV medicines in a single daily tablet.

The new case suggests that using stem cells with the CCR5-delta-32 mutation was a key factor, as prior attempts to stop antiretroviral therapy after transplants from donors with normal CCR5 genes were less successful.

A thematic analysis was carried out on data obtained from four HIV clinical trials conducted at KAVI-Institute of Clinical Research in Nairobi from 2009 to 2015.

"There is no virus there that we can measure".

"However, it also shows how far away we are from that point", he added saying it emphasized "the absolute importance of continuing to focus HIV prevention and treatment efforts".

The donor has two mutated copies of the CCR5 Δ32 allele, so the person is resistant to the HIV-1 virus strain that uses the CCR5 receptor since the virus can not enter the host cells.

Bone marrow transplant is a high-risk, life-threatening procedure. But she said his was also an unusual circumstance. and that the treatment is not practical for her patients with HIV.
