E-cigarettes are nearly twice as effective at helping smokers quit as nicotine replacement treatments like patches, lozenges and gum, according to the results of a major clinical trial.

The study, funded by The National Institute for Health Research and supported by Cancer Research UK, was published in The New England Journal of Medicine (citation below).

"We really isolated a very low-risk group of youth, and within that group experimentation with e-cigarettes had a pronounced effect on subsequent cigarette uptake", Stokes said.

In the United States and Britain, cigarette smoking is the leading preventable cause of death, taking some 480,000 lives each year in the USA and more than 100,000 in Britain. Quitting is notoriously hard, even with decades-old nicotine aids and newer prescription drugs. All of the smokers had gone to dedicated centres to help them quit.

Other experts, however, were far more critical.

"E-cigarettes may be a pathway to cigarette smoking, and a sizeable one", said senior study author Andrew Stokes of the Boston University School of Public Health. Last year, an influential panel of USA experts concluded there was only "limited evidence" of their effectiveness. "It does not support the unlimited availability of e-cigarettes".

For years, physicians have been reluctant to recommend e-cigarettes for smoking cessation because of a lack of clinical trial data, said Hajek. By the 52nd week, 18 per cent of the e-cigarette group was still off cigarettes, compared to 9.9 per cent of the standard treatment group.

A total of 886 participants underwent randomization. Both groups were also given at least a month's worth of weekly counseling sessions. Participants were responsible for buying follow-up supplies.

The problem with translating drug research to a consumer product like e-cigarettes is that the strict protocols of a drug trial (all participants use a certain drug at a certain dose and interval) may not translate to products that succeed by offering a wide range of possibilities to users.

"There is substantial evidence that they are less harmful than traditional cigarettes, but that doesn't mean they are not harmful". These effects were mostly mild.

"We need more studies about their safety profile, and I don't think anyone should be changing practice based on one study, " said Belinda Borrelli, a psychologist specializing in smoking cessation at Boston University. The American Cancer Society took a similar position past year.
